Community Integration Service (CIS) (https://www.awwa.org.sg/our-services/school-going-7-to-18-years-old/community-integration-service/) 7 to 21 years AAC, Fluency, Social communication, Speech Home-based, School-based and Centre-based (9 Lorong Napiri Singapore 547531) Contact: communityintegration@awwa.org.sg All referrals need to be accompanied with a report by a relevant medical professional stating a Speech disorder/ fluency disorder diagnosis. Child/youth must be diagnosed with a speech sound disorder/fluency disorder with relevant report(s) and have ongoing needs that impact on function/participation to meet the criteria for enrolment. The team may work on co-occurring social communication needs, but the primary goal remains to be targeting speech clarity/ AAC if needed in participation contexts meaningful for the client. Client needs to be attending mainstream primary/secondary school/specialised MOE institutions, Poly, ITE or University. Exclusion: SPED schools. Club Rainbow (https://clubrainbow.org) 0 to 20 years AAC, Feeding, Fluency, Language, Literacy, Social Communication, Speech, Voice Home-based, Centre-based: Blk 538 Upper Cross Street #05-263 Singapore 050538 Register as a beneficiary through a doctor’s referral Club Rainbow's programmes and services are offered to its beneficiaries only (children from ages 0-20 with chronic conditions). Submitted referrals by doctors are subjected to internal approval processes before they are accepted as beneficiaries and can enjoy access to our range of programmes and services. Examples of diagnosis accepted: GDD, muscular dystrophy, cerebral palsy, blood disorder, very low birth weight, rare genetic disorders.
